The ispection was
conducted by Clayton Wein of the Division of Reclamation, Mining and Safety (the Division). The site is
inactive; therefore, no company representative was present for the inspection.The weather was clear and sunny
with a temperature of 70 degrees farenheight. The ground conditions were mostly dry with a few damp areas from
recent precipitation.
HYDROLOGIC BALANCE - Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage – Acid and Toxic Materials 4.05.8; Impound ments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
A crumpled culvert identified in the June 2017 inspection of the site had been removed and placed next to
Ditch C during this inspection (Photo 1). The culvert had been excavated and the ditch was open during the
inspection. In the event of a precipitation event, water would be able to flow through this portion of the ditch
unobstructed (Photo 2).

Ditches A, B and C were clear of debris and capable of functioning as designed. The ditches were dry during the
inspection. During the inspection of Diversion Ditch B, the culvert buried underneath the haul road
appears to have minor damage at the outlet (Photo 3). Please monitor the culvert to ensure that the culvert
remains functional. No repairs are needed at this time.

There are two ponds at the Terror Creek Loadout, the dugout pond and the sediment pond (Photo 4). Both of the
ponds were dry at the time of the inspection. The embankments of the ponds were well vegetated and in good
condition. There were no indications of er osion or instability.

ROADS – Rule 4.03
Construction 4.03.1(3)/4.03.2(3)
Drainage 4.03.1(4)/4.03.2(4)
