Monitoring and analyses that are necessary to evaluate the situation and corrective actions
Sampling is not necessary because the chemical composition of the lime slurry is well known,
furthermore due to the thick nature and color of the slurry CC&V can ensure that the large bulk of
the slurry was removed before it could spread.
e. Results of the Operator's investigation
The root cause of the spill was a failure of one of the lime slurry pump housings. The bolt holding
the pump housing together sheared causing the pump housing to split in two, thus causing an
uncontrolled flow of lime slurry. The failure of the bolt was determined to be a one off
phenomena, the failure of the bolt was not caused by an over pressure or improper use of the
pump.
Should you require further information please do not hesitate to contact Tyler O'Donnell at 719.689.4056
or Tvler.O'Donnell a newmont.com or myself at 719.689.4055 or Me.Z..Burt n newmont.com.
Sincerely,
0
Meg Burt
Senior Environmental Manager
Cripple Creek and Victor Gold Mine
MB/tvo
ec: Tim Cazier—DRMS
Wally Erickson- DRMS
File
