Monitoring and analyses that are necessary to evaluate the situation and corrective actions <br /> Sampling is not necessary because the chemical composition of the lime slurry is well known, <br /> furthermore due to the thick nature and color of the slurry CC&V can ensure that the large bulk of <br /> the slurry was removed before it could spread. <br /> e. Results of the Operator's investigation <br /> The root cause of the spill was a failure of one of the lime slurry pump housings. The bolt holding <br /> the pump housing together sheared causing the pump housing to split in two, thus causing an <br /> uncontrolled flow of lime slurry.
