<br />1) Failure to maintain minimum requirements for legal compliance. Energy Fuels Coal, Inc. voluntarily dissolved <br />in December 2016. <br /> <br />2) Failure to maintain legal right of entry. <br /> <br />3) Failure to maintain mine and permit identification signs. <br /> <br />ROADS – Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3) <br />Drainage 4.03.1(4)/4.03.2(4) <br />Surfacing and Maintenance4.03.1(5) and (6)/4.03.2(5) and (6) <br />Reclamation 4.03.1(7)/4.03.2(7): <br />The road and road spur associated with the NW-MW well was inspected. The primary two track road has not yet <br />been reclaimed but has been self-healing due to lack of traffic. No erosion was observed during the length of either <br />road sections . The road spur has been reclaimed and vegetation along the corridor is thriving. <br /> <br />The reclaimed access road associated with the reclaimed substation pad was inspected. No erosion was visib le and <br />was in stable condition. Vegetation was sparse and the area was mainly populated with field bindweed <br />(Convolvulus Arvensis) which is labeled as a Class C noxious weed. The Division is requesting that Energy Fuels <br />Coal, Inc. to spray and monitor the area per the Southfield Mine PAP. <br /> <br />Bindweed located at substation road. <br />SUPPORT FACILITIES - Rule 4.04: <br />The NW-MW well pad and equipment storage pad located off the road spur were inspected. Both pads have been <br />reclaimed. The MW-NW pad is stable with mature vegetation growth. The equipment pad is still sparse with <br />vegetation but increase in vegetative density is noticeable. No erosion was observed along the equipment pad. <br /> <br />SIGNS AND MARKERS – Rule 4.02: <br />As noted in the Division’s previous inspection, the mine identification signs along th e County Road 15 had been <br />removed and was requested to be reinstalled. After the Division’s inspection on May 15, 2017, it was observed <br />that these signs were still missing and had not been reinstalled as per the Divisions request for compliance with
