Monitoring and analyses that are necessary to evaluate the situation and <br /> corrective actions <br /> Soil samples have been taken for the analysis of total, free, and WAD CN at the dump <br /> location and the area immediately adjacent to establish a baseline sample and a control <br /> sample. Final soil samples were taken below the excavated area to ensure effectiveness <br /> of recovery. <br /> All samples have been sent to an independent external laboratory for analysis and <br /> reporting. <br /> e. Results of the Operator's investigation <br /> The root cause of the spill was determined to be the inadequate inspection and clean-up <br /> of the 793F Haul Truck bed following use at LOB and prior to dispatching to the <br /> maintenance shop. <br /> Despite a focus on communication and personnel interaction, the investigation was <br /> unable to determine the reason for failing to inspect the truck bed. <br /> Other conditions and/or factors which contributed to this spill occurring were temperature <br /> extremes causing a frozen build up that later thawed midday allowing the material to <br /> release all at once. <br /> To prevent further occurrences of this type, a readdress of tailings haulage and <br /> inspection procedures was communicated with the crew on shift when it happened and <br /> will be communicated with future crews during a line out meeting, while cleaning <br /> methods and inspection techniques continue to be evolved. <br />
