If a <br /> Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br /> Reclamation Board will consider possible enforcement action. <br /> INSPECTION TOPIC: General Compliance With Mine Plan <br /> PROBLEM/POSSIBLE VIOLATION: Problem: The current mine plan may need to be updated and clarified <br /> pursuant to C.R.S. 34-32.5-112 (1)(c)(VI). The operator must provide sufficient information to describe or <br /> identify how the operator intends to conduct the operation within the approved permit boundary. The <br /> operator will need to re-survey and replace all permit boundary markers as described below, and provide DRMS <br /> with an accurate description of all activities taking place within and adjacent to the permit area, including the <br /> existing extent of excavated area(s). <br /> All DRMS permitted activity should be more than 200' from the existing gas well unless permittee has secured a <br /> damage waiver agreement as required. <br /> Operator is also required to salvage all available topsoil and stockpile/stabilize it for use in reclamation prior to <br /> excavation or covering. <br /> CORRECTIVE ACTIONS: If required after DRMS review of the provided information, the operator may be <br /> required to submit a Technical Revision, with the required $216 revision fee, to update and clarify the current <br /> approved mine plan to reflect existing and proposed activities by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 5/31/17 <br /> Page 1 of 4 <br />
