Timothy Cazier, P.E. <br /> Environmental Protection Specialist <br /> Colorado Department of Natural Resources <br /> Division of Reclamation, Mining and Safety <br /> Office of Mined Land Reclamation <br /> 1313 Sherman Street, Room 215 <br /> Denver, Colorado 80203 <br /> Re: Permit No. M-1980-244: Cripple Creek &Victor Gold Mining Company ("CC&V"); Cresson <br /> Proiect -Written notification — High Grade Mill CIP circuit release— 14th April 2017 <br /> Dear Mr. Cazier, <br /> In accordance with Rule 8, Section 8.2.3 of the Hard Rock, Metal and Designated Mining <br /> Operations Regulations (2015), CC&V hereby provides the Division with a formal report <br /> regarding the 100 Gallon High Grade Mill CIP circuit release that occurred on 14 April 2017. <br /> The following points addresses the requirements as outlined within Rule 8, Sub section 8.2. <br /> a. Actions taken to respond to and correct the emernency situation or condition <br /> • The down comer on Carbon in Pulp (CIP) tank number 8 became plugged resulting <br /> in the tank briefly overtopping which resulted in a minor volume of slurry (generally <br /> operating at a WAD CN concentration of 100 - 200ppm), estimated at less than 100 <br /> gallons of exiting the facility via the west overhead door. <br /> • Operators bypassed CIP tank 8 and unplugged the down comer, restoring normal <br /> operation to the circuit. <br /> • All slurry material that exited the HGM facility was collected and placed on the mill <br /> tailings pile to enable transport to the Squaw Gulch VLF (VLF 2). <br /> • Plant operators continued to monitor the CIP circuit to ensure further blockages did <br /> not occur. <br /> b. Any known or adverse impacts to human health, property or the environment <br /> Due to the minimal volume of slurry that exited the HGM (less than 100 Gallons) coupled <br /> with the immediate response time between the event and the remediation process, no <br /> �7 <br />
