Actions taken to respond to and correct the emernency situation or condition
• The down comer on Carbon in Pulp (CIP) tank number 8 became plugged resulting
in the tank briefly overtopping which resulted in a minor volume of slurry (generally
operating at a WAD CN concentration of 100 - 200ppm), estimated at less than 100
gallons of exiting the facility via the west overhead door.
• Operators bypassed CIP tank 8 and unplugged the down comer, restoring normal
operation to the circuit.
• All slurry material that exited the HGM facility was collected and placed on the mill
tailings pile to enable transport to the Squaw Gulch VLF (VLF 2).
• Plant operators continued to monitor the CIP circuit to ensure further blockages did
not occur.
b. Any known or adverse impacts to human health, property or the environment
Due to the minimal volume of slurry that exited the HGM (less than 100 Gallons) coupled
with the immediate response time between the event and the remediation process, no
7
