Exhibit D-Mine Plan Amendment IN <br /> 6.4.4(a) Description of the method(s)of mining to be employed in each stage of the <br /> operation as related to any surface disturbance on affected lands <br /> The Operator will initiate the following project activities in accordance with Mine Safety and <br /> Health Administration(MSHA)regulations and applicable state and county permit requirements. <br /> Phase 1 Construction and Reclamation Activities34 <br /> A. Access Road and Staging Area Construction: <br /> Three equipment staging areas that include a base of operations are planned. The three staging <br /> areas include areas in close proximity to the base of operation, and the May Day No. 1 and the <br /> May Day No. 2 benches. The base of operation will be constructed approximately 50 feet east of <br /> County Road(CR)-124. The base of operation will be the location where supplies and <br /> supporting outside service contractors(potable water delivery, augmentation water, fuel, oil <br /> products, mine equipment,porta-lets, emergency and safety equipment)will be delivered or <br /> serviced. <br /> The base of operation area(Exhibit D-Figure D-3)will not store regulated reportable hazardous <br /> or toxic chemicals on site. Fuel storage units have been designed with secondary containment <br /> capable of holding 150%of the volume of the stored product(See Exhibit U for containment <br /> details). The base of operation will include five employee and three visitor parking spaces, in <br /> addition to providing room to maneuver field and delivery trucks. <br /> A grass helicopter pad, as required by Fort Lewis-Mesa Fire District(FLFD),will be constructed <br /> and maintained to provide emergency medical evacuation services. The base of operation will be <br /> bermed and or fenced to limit access,to mitigate potential visual impacts;to reduce potential <br /> noise impacts and to improve site security. <br /> Equipment and ore staging areas will be located at the May Day No. 1 and 2 mine benches. The <br /> May Day No. 1 and No. 2 staging area will be used to temporally store underground mine <br /> equipment and supplies. The benches will also be used to temporarily store fuel (with secondary <br /> containment),mine rock,potable water, spill response kits, extinguishers, ore and a Port-a-let. <br /> B.
