SCHEDULE B TO SUBCONTRACT <br /> SUBCONTRACT SPECIAL PROVISIONS <br /> BY AND BETWEEN <br /> INTERSTATE HIGHWAY CONSTRUCTION, INC. <br /> AND <br /> AMES CONSTRUCTION,INC. <br /> IHC JOB#3097 <br /> 1. This is a Design-Build(DB) project. Quantities are approximate and will be subject to final design. Unit <br /> prices will not be subject to adjustment regardless of final in place quantities, with the exception of lump <br /> sum items carless agreed to per page 2, section 4 of subcontract. <br /> 2. In order to track pay item quantities and process pay estimates, Subcontractor will be required to provide <br /> Contractor a weekly report of completed items of work that can be verified and agreed upon by <br /> Contractor. This report will include locations, measurements and quantities. Contractor will pay only <br /> those items and quantities that are in compliance with all Contract requirements and have been accepted <br /> by Owner. <br /> 3. Subcontractor has included all mobilizations required to support its work. <br /> 4. Subcontractor will provide ear4iwo4.of base courses to +0.10' for concrete pavements, earthwork will <br /> conform with the contract drawings. Any balancing or rework as a result of proof-rolling, intermediate <br /> traffic or for other reasons shall be the responsibility of the Subcontractor. Excessive balancing or <br /> import of material will be the responsibility of the Subcontractor. IHC will define the limits of base <br /> placement to allow for the placement of IHC generated base coarse trimmings following finishing <br /> operations. If excess material remains at the completion of trimtning operations, and the base course <br /> has been properly placed to the limits defined b-y IHC, Ames will be reimbursed for the handling of this <br /> excess material on a time and materials basis. <br /> 5. Subcontractor will provide earthwork and base courses grade ready for asphalt pavements. <br /> 6. Subcontractor will perform structures, box culverts and or pipe in irrigation canals outside of the <br /> irrigation season. <br /> 7. Subcontractor shall be required to comply with the approved Quality Control Plan (QCP). <br /> • Required tests shall be performed by others at the intervals specified in the Contract or as directed by <br /> Owner. Expenses for any additional testing shall be assigned to the responsible party. <br /> • Contractors Quality Control Manager or his designated inspector shall provide inspection, if required, <br /> and documentation under the QCP. Any reinspection required due to deficient workmanship or <br /> materials shall be charged to the responsible party at $75 per manhour. <br /> 8. Contractor shall be responsible for initial layout and staking. Any re-staking or additional layout shall be <br /> the Subcontractor's responsibility. Subcontractor shall give a minimum 48-hour written notice of its <br /> survey requirements to allow for efficient coordination with the Surveyor and Contractor. <br /> 9. Alternate Item — Line A0320.1, A0320.2 and A0320.3 Muck Excavation are alternate methods of <br /> remediation of unstable soils that may or may not be used.
