Other conditions and/or factors which contributed to this spill occurring were temperature <br /> extremes causing a frozen build up that later thawed midday allowing the material to <br /> release all at once. <br /> To prevent further occurrences of this type, a readdress of tailings haulage and <br /> inspection procedures was communicated with all crews during a line out meeting, whilst <br /> cleaning methods and inspection techniques continue to be evolved. <br /> A standard of communication has been established between crews to prevent the <br /> unintended use of trucks in the pit before inspection.
