However, Amec provided CC&V a <br />letter, included in Appendix N, stating that no frozen material was placed during this <br />period. <br /> A portion of the Phase 1 Completion area was lined and had DCF in place at the time <br />that NewFields became the EOR. Amec was asked to provide verification that all DCF <br />that was placed within the Phase 1 Completion area was done so in accordance with the <br />project technical specifications. The acceptance that Amec provided is presented in <br />Appendix O.1; however, Amec was unable to provide documentation for the approval of <br />DCF placed on panels P‐1578 through P‐1615. NewFields contracted Leak Location <br />Services, Inc. to evaluate the condition of the underlying geomembrane within this area. <br />Leak Location Services concluded that the geomembrane was intact, with no <br />measurable damage. The results of the survey are provided in Appendix O.2. <br /> ECA had an insufficient supply of 80 mil LLDPE geomembrane to complete the Squaw <br />Gulch VLF Phase 1. The EOR confirmed that the use of 100 mil LLDPE (single sided <br />textured and smooth) geomembrane would be an acceptable substitute to complete the <br />last 16,000 square feet of Phase 1. The 100 mil LLDPE geomembrane meets all required <br />properties presented in the technical specifications and was previously approved for use <br />within the Squaw Gulch VLF PSSA. The QC documentation for the 100 mil geomembrane <br />used within the Squaw Gulch VLF Phase 1 Completion area is presented in Appendix I. <br />The 100 mil LLDPE geomembrane was placed at the south perimeter berm along the <br />high grade mill slope, in an area that ore is not expected to be stacked. Due to the size <br />and location of the area, the EOR believes that the installation of the 100 mil LLDPE <br />geomembrane accommodates the original intent of the design. <br />5. ENGINEER’S OPINION <br />Based on the construction activities observed, testing performed, and inspections completed, <br />NewFields certifies that the project was constructed in general accordance with the approved <br />design and technical specifications. <br />
