Cripple Creek & Victor Gold Mining Company
Squaw Gulch VLF Phase 1 Completion
Record of Construction Report
475.0106.006
October 14, 2016

P:\Projects\0106.006 CC&V CQA and EDS\J‐REPORTS\ROC Report\475.0106.006 CCV PHASE 1 ROC REPORT ‐ FINAL161014.DOCX
Page 11
permeability testing frequencies were one sample for every 4,000 cy. Twenty six SLF
samples were required to be tested for particle size distribution, atterberg limits,
laboratory compaction, and permeability testing. Twenty six SLF samples were tested
with passing results. SLF laboratory testing is summarized in Table 9 and individual test
results are presented in Appendix H.5.
The nuclear density/moisture testing frequency for SLF is one test for every 500 cy. Two
hundred and four SLF nuclear density/moisture tests were required. Two hundred and
sixty six SLF nuclear density/moisture tests were performed with passing results. The
depth check frequency for SLF is two checks for every acre. One hundred and twenty six
SLF depth checks were required. One hundred and thirty eight SLF depth checks were
performed with passing results. SLF nuclear density/moisture testing and SLF depth
checks are presented in Tables 11 and 12 respectively.
3.2.6. Drain Cover Fill
The Squaw Gulch VLF Phase 1 Completion area was constructed by placing 203,380 cy of
DCF. The particle size distribution and atterberg limits testing frequencies were one
sample for every 20,000 cy. Eleven DCF samples were required to be tested for particle
size and atterberg limits. Eighteen DCF samples were tested with passing results. DCF
laboratory testing is summarized in Table 10 and individual test results are presented in
Appendix H.6.
3.3. Geosynthetics Quality Control Submittals
The CQA Monitor reviewed and approved all geosynthetic QC submittals, including
geomembrane installation personnel résumés, geomembrane roll QC certificates,
geomembrane resin QC certificates, welding rod QC certificates, and geotextile QC certificates.
The CQA Monitor tracked all geomembrane delivered to site in the site inventory. The site
inventory for all geomembrane used within the Phase 1 Completion area are presented in
Appendix I.1.
3.3.1. Geomembrane Installation Personnel Résumés
ECA submitted the résumés of all installation personnel prior to construction or repair
activities within the Squaw Gulch VLF Phase 1 Completion area. The CQA Monitor
verified that the Installation Superintendent, Master Seamer and QC Inspector
possessed the installation experience required by the project technical specifications.
Geomembrane installation personnel résumés for all crews that performed work on the
Squaw Gulch VLF Phase 1 Completion area are presented in Appendix I.2.
