Monitoring and analyses that are necessary to evaluate the situation and <br /> corrective actions <br /> All slurry released to the lined area was excavated with a skid steer loader and <br /> recovered within the primary containment of the HPH (High pH)thickener. Impacted rock <br /> material was triple rinsed into the HPH containment sump pump and transferred into the <br /> HPH thickener. The oversized rock from the excavation was placed back into the spill <br /> excavation area after the rinse. <br /> Monitoring locations nearby the release site will continue to be monitored to determine <br /> any impact at the site. <br /> e. Results of the Operator's investigation <br /> The root cause of the event was identified as a failure of the CIP transfer pump. CC&V <br /> has repaired the pump that caused the event and continues to operate with a duty and <br /> stand by pump. <br /> Whilst the man door did not cause the event, its location limited the facilities containment <br /> of the spill. To enhance the containment this door is to be removed and will be replaced <br /> with a concrete berm that adjoins the existing structure walls, increasing the existing <br /> facility containment by an estimated 8 inches. This opportunity was identified prior to the <br /> event and a work order was already within the CC&V maintenance system. <br />
