Environmental Protection Specialist Colorado Department of Natural Resources Division of Reclamation, Mining and Safety Office of Mined Land Reclamation 1313 Sherman Street, Room 215 ��• _ c�� c�'�L_ Denver, Colorado 80203 Re: CC&V written notification - 25 gallon slurry release - 5th December 2016 Dear Mr. Cazier, In accordance with Rule 8, Section 8.2.3 of the Hard Rock, Metal and Designated Mining Operations Regulations (2015), CC&V hereby provide the Division with a formal report regarding the 25 gallon slurry release that occurred at operation on 51h December 2016. The event details provided by Meg Burt on Tuesday 6th December remain as initially reported as being whereby 25 gallons of slurry (31% solids), containing 670ppm WAD CN were released to secondary containment with the Mill platform. The following points addresses the requirements as outlined within Rule 8, Sub section 8.2. a. Actions taken to respond to and correct the emergency situation or condition As communicated within the initial notification, the mill operator promptly identified the issue with the sump box and shut down feed to the circuit. In an attempt to regain standard flow the operator attempted to back flush the pump however was unsuccessful at which time the operator changed to the backup CIP discharge pump and full flow was achieved. This action resulted in most of the release remaining within containment; however an estimated 25 gallons of slurry containing 670ppm WAD CN, flowed under a nearby access door and onto the mill secondary containment (lined area). The work area Supervisor was immediately contacted who coordinated the cleanup of released slurry. Clean up of the released material was completed within the shift and the area inspected by a member of the Environmental team who confirmed remediation activities were adequate.
