Actions taken to respond to and correct the emergency situation or condition
As communicated within the initial notification, the mill operator promptly identified the
issue with the sump box and shut down feed to the circuit. In an attempt to regain
standard flow the operator attempted to back flush the pump however was unsuccessful
at which time the operator changed to the backup CIP discharge pump and full flow was
achieved.
This action resulted in most of the release remaining within containment; however an
estimated 25 gallons of slurry containing 670ppm WAD CN, flowed under a nearby
access door and onto the mill secondary containment (lined area).
The work area Supervisor was immediately contacted who coordinated the cleanup of
released slurry. Clean up of the released material was completed within the shift and the
area inspected by a member of the Environmental team who confirmed remediation
activities were adequate.
