as per the review for the purpose of fulfilling the requirements of the Colorado Surface Coal Mining Reclamation <br />Act, and the Regulations of the Colorado Mined Land Reclamation Board for Coal Mining. The Division <br />requests that the permittee correct the deficiencies by submission of revised permit pages under appropriate <br />revision applications, or by activities on the ground, by 1 March 2017. <br />Item 1: Rule 2.03.4(1-4): <br />1. Please provide the addresses and % ownership of all officers and directors identified in permit section <br />2.03.4. <br />The rule states that social security numbers and employer identification (EID) numbers shall also be <br />provided. <br />2. Please provide EID numbers as specified or provide revised pages referencing the location of this <br />information in the permit. <br />3. Please also provide each person's percentage of ownership. <br />Item 2: Rule 2.03.4(6-9): <br />Map 1 shows the location of surface owners and parcels. However, addresses are not provided for each of these <br />owners as required by the rules. <br />4. Please provide valid and proper mailing addresses for each owner and leaseholder as revised pages for <br />incorporating into the permit. <br />5. Also please provide as a revised page the valid and proper mailing address for the North Central Energy <br />Company identified as the owner of the subsurface coal on Map 2. <br />Item 3. Rule 2.03.5(1-3): <br />The current Golden Eagle permit lists a series of Notice of Violations (NOV's) for the New Elk Coal Company. <br />It is unclear that these violations pertain to Golden Eagle. <br />6. Please either, clarify what NEEC mines relate to these NOV's and submit revised page to correctly <br />indicate this information or, remove them from the permit if they are not pertinent to Golden Eagle. <br />Item 4. Rule 2.03.9(1-4) <br />The permit refers to an Exhibit 3 as the Certificate of Insurance submitted in November of 2001 in the <br />confidential section. Also, there are numerous Exhibit 3's referring to Drill Holes. <br />7. Please clarify what the status of the numerous Exhibit 3's are. If the Exhibit 3 referencing the Certificate <br />of Insurance in the confidential section is no longer relevant, please submit revised pages to remove this <br />from the permit. <br />In Volume one of the permit there is an Exhibit 3 with the Certificate of Liability Insurance dated expiring in <br />September of 2013. <br />8. Please provide an updated Certificate of Liability Insurance for inclusion in the permit. <br />Please update the following as they are outdated and not reflective of the permit at this time and provide <br />to DRMS as revised pages: <br />a.
