PERMIT #: M-2000-052 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: November 10, 2016 <br /> <br /> <br />Page 2 of 5 <br /> <br />OBSERVATIONS <br /> <br />This was a surety related inspection resulting from an Operator request to enter Temporary Cessation (TC) <br />received by the Division on October 25, 2016. The Operator (Rocky Mountain Materials & Asphalt, Inc.) was <br />represented by Mr. Rusty Cochrane who was present for the inspection. <br /> <br />The Coyote Ridge site access road entrance is located approximately 4 miles south of Florence, off State Hwy <br />67. This is a 112c sand and gravel mine. The mine was not operating at the time of the inspection. <br /> <br />Inspection: <br />Markers & Boundary – A permit sign was displayed at the site entrance (see Photo 1). This is a ridge top <br />operation and mining activity is restricted by topography. <br /> <br />Mining Plan – No deviations from the approved mining plan were observed. Little activity appears to have <br />occurred since the previous inspection, February 19, 2015. <br /> <br />Reclamation – The over steepened highwalls observed during the previous inspection in Area C (reference Figure <br />1) were confirmed to have been flattened to 1H:1V or flatter (see Photo 2). A review of 2002 Google Earth <br />images indicated disturbances in Areas A (see Photo 3) and B (see Photo 4) that appear to have adequate <br />reclamation. The assumption of adequate reclamation for these areas were confirmed in the field during this <br />inspection: 1.4 acres of the 5.2 disturbed acres in Area A, and all 1.1 acres in Area B (reference Figure 1). Mr. <br />Cochrane was informed of this during the inspection and this Specialist recommended RMMA request a release <br />for these two areas, and if the Operator wishes to completely remove them from the permit area, these areas <br />will need to be marked so that no future mining activity will re-disturb the areas. <br /> <br />Environmental – No problems with erosion/sedimentation or noxious weeds were observed. <br /> <br />Records: <br /> Revisions: Technical Revision 1 (TR-01) was approved in 2013 to eliminate the phased mine plan. <br />Temporary Cessation (TC-01) was requested on October 25, 2016 and met the requirements of Rule <br />1.13.5(2). <br /> Commitments/Stipulations: None <br /> Other Records <br />a) The total permitted area is 193.280 acres, with a maximum allowed disturbed area (at one time) of <br />45 acres. <br />b) A review of the approved reclamation plan states “Mine slopes will generally be excavated to a 1:1 <br />slope.” <br />c) The previous inspection was performed on 2/19/2015. One problem for overly steep highwalls was <br />cited and subsequently resolved. <br />d) The anniversary date is September 11. Annual reports and fees are current through 2016. The <br />reported affected area for the last two annual reports has been a consistent 8 acres. <br />e) The post-mine land use is rangeland. <br /> <br />Bond: <br />The Division currently holds a $32,400 bond which is considered adequate for the current disturbed area (7 +/-
