PERMIT #: P-2009-025 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: September 15, 2015 <br /> <br /> <br />Page 3 of 5 <br />denied by the MLRB in October 2015. <br /> Commitments/Stipulations: The contents and cover of the backfilled mud pits will not exceed the <br />following limits: <br />1) The concentration of radium 226 or radium 228 may not exceed the background level by more <br />than 5 picocuries per gram (pCi/g). <br />2) The concentration of natural uranium with no radioactive decay products present may not exceed <br />the background level by more than 30 pCi/g. <br />3) Background levels shall be determined by sampling each proposed mud pit location prior to any <br />disturbance associated with the proposed drilling activity. <br /> Other Records <br />a) Well permits. The Division does not have copies of the well permits for monitoring wells MW-27- <br />290 through 294. These are required to ensure compliance with regulations. The lack of well <br />permits submitted to the Division is cited as a problem on page 2 of this report. <br />b) The total permitted area is 4.5 acres. <br />c) The previous inspection was performed on 10/20/2011. No problems were cited. <br />d) The anniversary date is October 7. Annual reports and fees are current through 2015. <br />e) Borehole abandonment tracking records are attached. <br /> <br />Bond: <br />The Division currently holds a $154,936.58 bond. This was reviews with the approval of MD-02 and is deemed <br />adequate. <br /> <br />Summary and Recommendations: <br />1. Submit well permits for monitoring wells MW-27-290 through 294 by the Corrective Action Due date on <br />page 2 of this report. <br />2.
