Number of Partial Inspection this Fiscal Year: 2 <br />Number of Complete Inspections this Fiscal Year: 1 <br /> <br /> Page 5 of 6 <br /> <br />to the permit, this old culvert spillway was to have been removed. After the Division’s July 2016 <br />inspection, the Division required PSCM to complete one of the follo wing corrective actions: <br />i. Remove the original corrugated metal culvert spillway that was decommissioned, and <br />backfill and repair the embankment in accordance with the approved design. This activity <br />was required to be complete by August 12, 2016 <br />Or, <br />ii. Submit a Technical Revision to update the design of Pond 004 if necessary to comply <br />with the Division of Water Resources requirements. The revision must be received by the <br />Division by August 12, 2016. <br />iii. PSCM has completed the required repairs to the embankment . The Division had extended <br />the deadline to complete this activity to August 26, 2016. PSCM submitted photographs <br />of the work completed prior to the deadline. This item is considered addressed. <br />b. Please submit a Minor Revision to update Map 2.04.7-M1 to depict the location of th e new stock- <br />tank installed above Pond 004 in accordance with Rule 2.010.3(1)(e). <br />i. This item remains outstanding. <br />c. Clean out the upper reaches of ditches FA-D5 and FA-D6. <br />i. This item remains outstanding. <br />d. Repair the section of the FA-D6 ditch that has buckled. <br />i. This item remains outstanding. <br />e. Remove and/or otherwise treat the tamarisk trees that have established within this seep area. <br />i. This item remains outstanding. <br />f. Culvert SC-27 is about half full of sediment at the inlet and will need to be cleaned. <br />i. This item remains outstanding, but it does not appear the culvert has gotten much worse <br />since the last inspection. <br />g. The disturbance boundary markers at the southern end of the disturbed area have been knocked <br />down by cattle. According to Mr. Watterson, these carsonite markers require a special tool to <br />place them into the ground. He said he needs to acquire one before he can re-set the markers. <br />Once the tool has been acquired, please replace these markers. <br />i. This item remains outstanding. According to Mr. Watterson, the tool has been delivered <br />and the markers will be replaced in the near future. <br /> <br /> <br />TOPSOIL – Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br /> <br />The topsoil stockpile was observed.
