August 17, 2016 C-1981-018/Deserado Mine CCW <br /> <br /> <br />Number of Partial Inspection this Fiscal Year: 2 <br />Number of Complete Inspections this Fiscal Year: 0 <br /> <br /> Page 4 of 8 <br /> <br />- 12L Vent Shaft <br />- Nitrogen Plant #2 <br />- RDH-4 <br />- The Water Storage Tanks <br />- The Radio Tower <br /> <br />The berms at the SAEs were intact and functioning as designed. There were no erosional features identified at any <br />of the SAEs. The gravel pads at the sites were in good condition. No major rutting was observed. <br /> <br />Each of the degas boreholes were marked with an identification sign. The topsoil from the pads was stripped and <br />placed in a pile on the edge of each pad. The topsoil piles were marked with a wooden post. There were metal <br />fences put up around each of the boreholes. <br /> <br />The pads to the 11L-TG-V1, 12L-XC-53 and 12L-XC-67 nitrogen injection wells are in good condition. The <br />gravel placed on the pads has not been over compacted. Each of the sites is marked by an identification sign. The <br />topsoil for the pads has been placed in a pile on the edge of the pads, marked by a wooden post. <br /> <br />TOPSOIL – Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br /> The topsoil pile associated with the Facilities Area was inspected. The pile has a cover of well-established <br />vegetation on it (Photo 4). There were no indications of erosion or instability at the pile. The perimeter ditch for <br />the pile is a riprap lined channel (Photo 5).
