(PSCM). I, Jared Ebert of the Colorado Division of Reclamation, Mining and <br />Safety (Division) conducted the inspection. Mr. Brian Watterson was aware of the inspection but did not <br />accompany me during the inspection. Ms. Miranda Blomquist accompanied me to Pond 004 but did not <br />accompany me during the rest of the inspection. The weather was partly cloudy and warm during the inspection. <br />EXPLOSIVES – Rule 4.08 <br />Distance Prohibitions 4.08.4; Warnings 4.08.4; Control of Adverse Effects 4.08.4: <br />The explosives storage area was observed. No problems were noted. <br /> <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage – Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> <br />Pond 004 was observed. During the Division’s May 2016 inspection, it was observed that water was discharging <br />from an old culvert spillway that was decommissioned back in 2005. According to the permit, this old culvert <br />spillway was to have been removed. After the Division’s July 2016 inspection, the Division required PSCM to <br />complete one of the following corrective actions: <br /> Remove the original corrugated metal culvert spillway that was decommissioned, and backfill and repair <br />the embankment in accordance with the approved design. This activity was required to be complete by <br />August 12, 2016 <br /> <br />Or,
