PERMIT #: M-1981-185 <br />INSPECTOR’S INITIALS: LJW <br />INSPECTION DATE: June 30, 2016 <br /> <br /> <br />Page 4 of 7 <br />General Site Observations <br /> <br />The overall condition of the site was free from excessive trash and debris. However, areas of severe <br />infestation of thistle and other noxious weeds were observed throughout the site particularly on the access <br />roads and berms. The noxious weed problem is an unresolved problem cited in the October 22, 2015 report. <br />This is once again being cited as a problem with a compliance date of September 1, 2016. The main <br />concentration of the infestations are located at the May Day 2 area as well as along the road to the May Day 1 <br />area. Photo Six shows an example of the area of concern. Due to the timing of the observation, the Division is <br />requiring the IMMEDIATE treatment of noxious weeds within the disturbed a rea to prevent further spreading. <br />The operator needs to submit photo documentation of the treatment, a map showing the areas that were <br />treated and a report from the sprayer noting the specific chemical and application rate used during the <br />treatment.
