Mr. Jack Henris <br />July 13, 2016 <br />Page 3 <br />m:\min\tc1\_teller\m-1980-244 cc-v\am-11\m-80-244-am-11maincommentr12016-07-13.docx <br />A quick check using Google Earth satellite imagery suggests some structures may have <br />been overlooked, for example there is a structure about 850 feet west of Grassy Valley <br />monitoring wells GVMW-22B & GVMW-22A along Beaver Valley Rd <br />(38°44'26.24"N, 105° 6'51.04"W) that is almost certainly within the affected area <br />boundary. Be aware that wells, fences, streets and roads are permanent man-made <br />structures that need to be identified in accordance with Rule 6.4.3(g). Please revise the <br />map to include a background such as that used for Drawing C-4b, so that an adequate <br />inclusion of appropriate structures can be evaluated. Also, please double check <br />structures in the area to be sure all appropriate structures are included. <br />c. TR-69 and AR-01 were submitted and approved to address the new radio tower location. <br />Drawing C-1a appears to show the radio tower (in Section 30) is now outside the <br />permit/affected area boundary. Please provide an explanation. <br />d. R0055878 CC&V Pavilions – this drawing shows this parcel (in Section 29) is partly <br />inside and partly outside the permit/affected area boundary. If mining related activity <br />is conducted in this area, it must be included in the affected area boundary. Please <br />provide an explanation. <br />5. Drawing C-3. There are numerous utility alignments identified on this drawing [Rule <br />6.4.3(b)]. Pursuant to Rule 6.4.20(c)/Exhibit T the Division may require statements from <br />utilities “that the mining and reclamation activities, as proposed, will have "no negative <br />effect" on their utility”. Please identify or distinguish which buildings and utilities are <br />owned by CC&V and which are owned by another entity. <br />6. Drawing C-4. This drawing intended to show proposed facilities shows the historic Chicago <br />Tunnel underground workings but does not include the proposed underground workings <br />shown in Drawings C-5a and C-5b. Please show these proposed workings for overall site <br />reference and indicate where shafts daylight. <br />7. Drawing C-4a. This drawing intended to show changes in disturbance categories but does <br />not indicate a change in disturbance where the proposed underground workings shafts (as <br />shown in Drawings C-5a and C-5b) daylight. Please indicate a change in disturbance <br />category where these shafts daylight. <br />8. Drawing C-5a. <br />a. The legend shows a line color for 2016, 2017 and 2018, but there are no “2020 <br />Sectional View” on the drawing. Please provide an explanation. <br />b. There is a discrepancy between the title block scale (1” = 2000’) and the scale bar <br />(1 inch = 200 ft). Please provide a corrected drawing. <br />c. Sectional views for 2016, 2017 and 2018 show purple and orange lines that are near <br />vertical shafts. Please show on the plan views where these shafts daylight. <br />9. Drawing C-5b. <br />a.
