PART 1: GENERAL <br /> 1.1. Summary <br /> This SPECIFICATION describes requirements for the manufacture, fabrication (if needed), supply, <br /> and installation of polyethylene geomembrane for lining of the valley leach facility, as shown on <br /> the DRAWINGS; and construction quality control monitoring and testing. All procedures, <br /> operations, and methods shall be in strict compliance with the SPECIFICATIONS, Construction <br /> Quality Assurance Plan, and the DRAWINGS. <br /> 1.2. Submittals <br /> A. CONTRACTOR shall provide the following information relating to the geomembrane <br /> MANUFACTURER and INSTALLER with its proposal. <br /> 1. Information from the MANUFACTURER including company name, address,telephone <br /> number, the names of the company president and quality control manager, and <br /> narrative of the company history. <br /> 2. Information from the MANUFACTURER on factory size, equipment, personnel, <br /> number of shifts per day, and capacity per shift. <br /> 3. Quality Control Manuals from the MANUFACTURER and INSTALLER for the <br /> installation and testing of the geomembrane, including trial seams, seaming, <br /> nondestructive testing, destructive testing procedures, repair procedures and in- <br /> field quality control forms. Upon review of the Quality Control Manuals, MANAGER <br /> may request additional testing during the manufacturing process at no additional <br /> cost to CC&V. <br /> 4. A list of at least five completed facilities from the MANUFACTURER totaling a <br /> minimum of 5 million (5,000,000) square feet between all five facilities of the type of <br /> geomembrane that is being installed at the Cresson Project. Each entry in this list <br /> should specify the name and purpose of the facility, its location and date of <br /> installation, the name of the owner, the project manager, designer, fabricator (if <br /> any), and the geosynthetics installation contractor and the name and telephone <br /> number of the contact at the facility who can discuss the project. In addition, the <br /> geomembrane thickness and total square footage of the installation surface should <br /> be included. <br /> 5. Installation capabilities of the INSTALLER. <br /> 6. Information on equipment and personnel from the INSTALLER. <br /> 7. A list of at least five completed facilities, totaling five million (5,000,000) square feet <br /> for which INSTALLER has installed the type of geomembrane that is being installed at <br /> the Squaw Gulch VLF Project.
