Newmont Mining Corporation <br /> Cripple Creek&Victor Gold Mine . <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,June 18,2016 <br /> required project technical specifications, prior to accepting the surface for geomembrane <br /> installation. All test locations were surveyed by Ames. <br /> 1.6 Drain Cover Fill (DCF) <br /> Drain Cover Fill (DCF) placement was performed in the areas shown on Figure 4. CAT 740 haul <br /> trucks hauled DCF from the stockpile to the Phase 1 VLF. The DCF was placed in a 2-foot lift on <br /> previously approved geomembrane by low ground pressure CAT dozers. Haul routes consisted <br /> of 4-foot lifts that will be spread into 2-foot lifts at a later date. Ames' laborers were present <br /> during placement to assist in removing debris/sandbags, installing high volume solution <br /> collection piping, and ensuring that the lift thickness was correct. A NewFields' representative <br /> constantly monitored the DCF placement. <br /> 1.7 Riprap <br /> No work performed during this reporting period. <br /> 2.0 GEOSYNTHETICS <br /> 2.1 Geogrid <br /> No work performed during this reporting period. <br /> 2.2 Geomembrane Installation <br /> Geomembrane installation continued within the Phase 1 SGVLF in the areas shown on Figure 3. <br /> Approximately 203,000 square feet (P-1796 to P-1865) of 80 mil LLDPE (double sided texture) <br /> was deployed. Subgrade was inspected and approved by NewFields and ECA representatives <br /> prior to geomembrane deployment. Approximately 10,152 linear feet of seam was fusion <br /> welded using two fusion welding machine/operator combinations. Twenty-three destructive <br /> testing samples were marked during fusion seaming. Approximately 1,562 linear feet of seam <br /> was extrusion welded using one extrusion welding machine/operator combinations. Six <br /> destructive testing samples were marked during extrusion seaming. All destructive tests <br /> performed met project specifications. Non-destructive testing was performed for fusion welded <br /> seams. Repairs were performed using the extrusion welding method for all marked defects. <br /> Vacuum testing was performed for all extrusion welded seams and repairs. <br /> 2.3 Anchor Trench <br /> Approximately 1,050 feet of permanent anchor trench was excavated along the Phase 1 <br /> perimeter in accordance with design drawings and project technical specifications. <br /> Page 3 <br />
