PERMIT #: M-2016-010 <br />INSPECTOR’S INITIALS: AME <br />INSPECTION DATE: April 12, 2016 <br /> <br /> <br />Page 4 of 14 <br />below ground utilities, irrigation ditches, maintained or public roads, bridges, railroad tracks, cemeteries, <br />communication antennas, pipelines, water wells, water storage structures, discharge and conveyance structures, <br />etc. The applicant should be sure to provide the Division with agreements for all such structures located inside <br />of or within 200 feet of the proposed affected land. <br /> <br />The Division observed that public notices signs had been placed at the current site entrance where Little Turkey <br />Creek Road intersects Hwy 115 (Photo 18), and at the proposed site entrance off of Hwy 115. Considering that <br />Hwy 115 is the only road located near the proposed site where the notices may be viewed by the public, the <br />locations chosen appear to be adequate. The other sides of the proposed permit area are either not accessible or <br />are only accessed via private roads on private property. According to the photos provided with the application, <br />the signs originally listed the public comment period to end on April 11, 2016. This date is typically estimated <br />in the beginning, as the correct date is not known until the application is considered filed and the date of the 1st <br />newspaper publication is known. Once this information was known, the applicant corrected the date on the signs <br />to read April 19, 2016. This correction had been made by the time of the inspection. <br /> <br />The proposed affected land, which appears to be the same as the proposed permit area in this case, has not yet <br />been delineated on the ground. The Division recommends the applicant mark this boundary in accordance with <br />Rule 3.1.12(2), which states that the boundaries of the affected area shall be marked by monuments or other <br />markers that are clearly visible and adequate to delineate such boundaries. Additionally, per Rule 3.1.12(1), <br />once the permit is issued, the applicant shall post a sign at the main entrance to the mine site which shall be <br />clearly visible from the access road. The permit identification sign should include the following information: <br />name of the operator, a statement that a reclamation permit for the operation has been issued by the Colorado <br />Mined Land Reclamation Board, and the permit number. <br /> <br />Due to inclement weather, the Division was unable to conduct a full inspection of the site. Therefore, the <br />Division discussed with the applicant the need to conduct an additional preoperation inspection at a later date. <br /> <br /> <br />
