PERMIT #: M-2008-018
INSPECTOR’S INITIALS: ACY
INSPECTION DATE: May 11, 2016

stockpiles approximately are 15’ high or less were observed. Slopes range from near vertical to 2:1. Overall
there is no excessive erosion of threat for off-site damage. The southwestern portion of the pit has a berm to
separate between the pit floor and the hillside.

The middle stockpile area has four main stockpiles that are approximately 15’-20’ tall and vary volume. There
is some vegetation on these piles. The floor is stable with a combinat ion of weeds and native vegetation. All
slopes are stable and appear to be 2:1 to less. A small trench is excavated to the east of the stockpiles and is
approximately 8’ deep.

The eastern portion is largely undisturbed. A two track runs along the fence a nd connects with the ranch road
along the norther permit boundary. There is also a more distinct road cut along the contour that divided the
eastern and western portions of the pit. A dump site is situated along the northern edge of the middle road
and permit boundary. There is a test pit in the undisturbed eastern portion that is approximately 10’x10’.

The site is currently not active. No equipment or fuel is stored on site. Based on conversations with the
operator, the site has been inactive since 2008. In 2014 the site was transferred back to the landowners and
has remained inactive ever since. The abundance of vegetation on stockpiles supports these statements. This
is cited as a problem. The operator shall place the permit into Temporary Cessation (TC) per rule 1.13 or begin
final reclamation. Attached to this report is ‘Temporary Cessation Notice’ form.

The permit stipulates concurrent reclamation will be practiced with the 3 acre processing area being that last
to be reclaimed. It does not appear that any reclamation has occurred to date. Currently approximately 20
acres are affected by mining. The current bond is based off of this 3 acre assumption.

Currently the Division holds a bond of $12,681. No surety increases have occurred since the permit i ssuance.
In an effort to ensure the Financial Warranty adequately, reflects the actual current cost of fulfilling the
requirements of the approved reclamation plan the Division will be updating the reclamation cost estimate. A
copy of the staff calculations will be sent under separate cover.

Responses to this inspection report should be directed to: Amy Yeldell at the Division of Reclamation, Mining
and Safety, 1313 Sherman St., Room 215, Denver, CO 80203.
