PERMIT #: M-2008-018 <br />INSPECTOR’S INITIALS: ACY <br />INSPECTION DATE: May 11, 2016 <br /> <br /> <br />Page 2 of 10 <br />INSPECTION TOPIC: Signs & Markers <br />PROBLEM/POSSIBLE VIOLATION: Problem: The mine identification sign and affected area boundary markers <br />were not observed per the requirements of Rule 3.1.12. <br /> <br />CORRECTIVE ACTIONS: The operator must post a sign at the entrance to the mine site which shall be clearly <br />visible from the access road and specify the following; the name of the operator, indicate that a reclamation <br />permit for the operation has been issued by the Colorado Mined Land Reclamation Board, and the permit <br />number. The boundaries of the affected area must be marked by monume nts or other markers that are clearly <br />visible and adequate to delineate such boundaries. <br /> <br />CORRECTIVE ACTION DUE DATE: 7/29/16 <br /> <br /> <br /> <br /> <br /> <br />OBSERVATIONS <br /> <br />This inspection was conducted as part of the Colorado Division of Reclamation, Mining and Safety normal <br />monitoring program. The Flying Triangle Ranch Pit is a 112c permitted site that includes a total of 61 permitted <br />acres of which approximately 20 acres appear disturbed. The site is located approximately 41 miles northwest <br />of Craig, Colorado. It is located just south of the state line and the nearest town is Baggs, WY. The site is <br />accessed from Moffat County Road 4. No representative was present during th e inspection however Cliff <br />Henderson of Flying Triangle Ranch, LLC was contacted prior to the inspection. Amy Yeldell of the Division <br />conducted the inspection. <br /> <br />The site was accessed through the Flying Triangle Ranch. It is noted that the permit maps indicate the <br />entrance is on the south side of the pit and goes through State Land Board property. No sign was posted at <br />either of the entrances. This is cites as a problem. The permit boundary is delineated by a fence on the south <br />side and appears to align with farm roads. Little to no actual markers were observed. This is cites as a <br />problem. The operator shall post a mine sign and delineate the permit boundary per rul e 3.1.12. Please <br />provide the Division with photo documentation by the corrective action date. <br /> <br />The 2016 annual report stated N/A for weed treatments. The operator is reminded that all state listed noxious <br />weeds shall be treated within the permit boundary. No large infestations were observed however a few <br />annuals were starting to emerge. <br /> <br />Lots of miscellaneous metal and other debris not related to mining was observed. Farming/ranching trash and <br />debris may not be stored or disposed of within the mining permit boundary. This is cited a problem, please <br />remove all debris not related to mining from the permit boundary. Also all miscellaneous metal dumbs shall be <br />removed regardless of their use or placed in a secondary containment. <br /> <br />The site is comprised of three main areas. The western excavated pit, the middle stockpile area and the <br />eastern undisturbed area. <br /> <br />The western pit area has excavated highwalls are approximately 8’-15’ high. The pit floor has been excavated <br />to an average of 10’ below grade. One large stockpile is approximately 20’ high and several other smaller
