Newmont Mining Corporation <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,May 21,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> • Confirmatory drilling was performed at the Upper Anaconda Mine Complex (further <br /> remediation required). NewFields personnel observed confirmatory drilling activities <br /> and noted any voids that were encountered. <br /> • A portion of the fifth step of the lower Anaconda Mine Complex geogrid had previously <br /> been removed so that the third layer of geogrid could be placed on the fourth step. New <br /> geogrid was tied into the existing three layers of geogrid at the fifth step. As built survey <br /> was completed prior to the placement of six inches of select structural fill over each <br /> geogrid layer. Compaction of the select structural fill was achieved using a smooth drum <br /> roller. The lower portion of the Anaconda Mine Complex is considered remediated; <br /> however, the upper portion requires further investigation. <br /> • An excavator worked to remove the shot rock at UG#6477 and UG#6478. Once <br /> competent rock was encountered, the working was backfilled per project specifications. <br /> The area is being be prepped for a concrete plug and a geogrid cap. Further remediation <br /> is required. <br /> • Blasting was performed at the Upper Anaconda Mine Complex. After the blast, an <br /> excavator removed the shot rock so that the working could be further investigated. <br /> Further remediation is required. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint above the <br /> fourth geogrid step at the lower Anaconda Mine Complex. NewFields personnel inspected fill <br /> material and observed placement and compaction procedures to ensure that they were in <br /> accordance with the project technical specifications. Ambient temperatures remained above 32 <br /> degrees Fahrenheit during fill placement. <br /> 1.3 Subgrade <br /> No work performed during this reporting period. <br /> 1.4 Leak Detection Trench <br /> No work performed during this reporting period. <br /> Page 2 <br />
