Skies were partly cloudy and a light wind blew. Temperatures hovered at 50 degrees
F. The mid term review was discussed and DRMS requested that an application for a minor revision be submitted
to DRMS. Upcoming vegetation sampling dates, protocols and methodology were also discussed.

Maintenance items from the March 2016 inspection were revisited and discussed. The records check was deferred
to next month so that documents and maps discussed today could be updated and a complete inspection conducted.
PERMITTING UPDATE
A midterm review of the permit was conducted by DRMS and submitted to the operator in January of 2016.
DRMS received a response in February of 2016 and is still awaiting an application for a minor revision from
the operator and, or Mr. Mike Savage to address topics resulting from the review.

MAINTENANCE ITEMS OUTSTANDING from PREVIOUS INSPECTION
March 2016

1. Repair fallen marker at the west edge of the permit as per Rule 4.02.
2. Map 5-7 boundary discrepancies will be corrected. To resolve this item, please submit
an updated map to DRMS, as per Rules 2.10.1 and 2.10.3. Also, place one in the
records at Goff Engineering.
3. The slump at south- southeast slope of stockpile #4 appeared stable and it
appeared that appropriate vegetation was greening and filling in. This item is
resolved.
4. Transmitted debris along the Ewing Mesa Road as shown in Photo 3 must be mitigated
as per Rule 4.03.2. Sediment movement should be monitored and the source
area of the debris identified and stabilized.
5. On east side of the Southeast Pasture Road identify source area of the debris ,
stabilize and mitigate. Also, runoff channel should be stabilized and mitigation
performed as per Rule 4.03.2. (Photo 4).
