PERMIT #: M-2007-006
INSPECTOR’S INITIALS: TOD
INSPECTION DATE: January 14, 2016

Gen. Compliance With Mine Plan:
The operation appeared to be following the approved mine plan. All mining activity appeared to be within the
approved permit boundary. The Mining operation has not affected more than 25 acres. There was also a scale
house located at the entrance to the pit.

Roads:
The access road was stable, it appears that the access road has not been used recently. All internal pit haul roads
appeared to be stable.

Revegetation:
There was plant growth throughout Phase 1, and the areas disturbed by the previous mining operations. With the
exception of the areas seeded under the 111 permits, the growth in phase 1appeared to be volunteer vegetation,
comprised of native vegetation and annual weeds, thus filling in most of the pre-law disturbance. The two 111
permits appeared to have been seeded, desired vegetative cover has begun to establish. Tamarisk (salt cedar) trees
and Russian Olive trees are present Throughout Phase 1. The Tamarisk and Russian olive trees have become
established in phase 1. The Tamarisk and Russian olive trees appear to be primarily in the pre-law disturbed areas
and not the areas currently affected by the current mining operation. When completing final reclamation the
Operator will need to treat and remove all noxious weeds.

Erosion/sedimentation:
There were a few rills and gullies beginning to form on the slopes of phase 1, all storm water and sediment appears
to drain into the pit and be captured within the pit.

Signs and Markers:
A mine I.D. sign was located at the entrance to the mine site. The permit boundary was marked by t-posts and a
barbwire fence.

CONCLUSION

It appears that minimal mining activity has taken place under the current issued permit. It appears that most of
phase 1 has been affected by previous mining operations. Based on the approved Mine Plan, it appears that most
of Phase 1 will be re-mined and reclaimed once activities began under the approved permit. Once Phase 1 is
mined under the current permit many of the pre-law areas will be reworked and reclaimed, thus resolving the
issues with noxious weeds and minor erosion.

Inspection Contact Address
John Paul Ary
Fremont Paving & Redi-Mix, Inc.
839 Mackenzie Ave
Canon City, CO 81215

cc: Wally Erickson, DRMS
