Newmont Mining Corporation <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,April 23,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> • NewFields personnel observed confirmatory drilling activities and noted any voids that <br /> were encountered. Confirmatory drilling was performed at the following underground <br /> workings: <br /> o UG#6477 (further remediation required) <br /> > After the subgrade was accepted, three layers of geogrid were placed on the fourth <br /> "step" for UG#6617. As built survey was completed prior to the placement of six inches <br /> of select structural fill over each geogrid layer. Compaction of the select structural fill <br /> was achieved using a smooth drum roller. Compaction of the select structural fill is <br /> required before the working can be considered remediated. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint. NewFields <br /> personnel inspected fill material and observed placement and compaction procedures to <br /> ensure that they were in accordance with the project technical specifications. Ambient and fill <br /> material temperatures were monitored and recorded during fill placement to ensure that no <br /> frozen material was placed or compacted in the area. The following structural fill construction <br /> activities occurred during the shift: <br /> • Slope rough grading (cut to stockpile/hauled to fill) in areas bounded by bench stations: <br /> o K20+00 to K22+00 and S25+00 to S27+00 <br /> • Slope Contouring (cut to fill) in areas bounded by bench stations: <br /> o M12+00 to M14+00 and K16+00 and K18+00 <br /> • Compaction of Structural Fill was achieved using a smooth drum roller. <br /> 1.3 Subgrade <br /> Subgrade was accepted and approved for geogrid installation at the fourth "step" of UG#6617. <br /> 1.4 Leak Detection Trench <br /> No Leak Detection activities performed during the reporting period. <br /> Page 2 <br />
