SECTIONTWO General Personnel Information <br /> Successful operation and maintenance of 3 Dam requires a quality-oriented,well-trained staff. <br /> Presented below are guidelines and/or requirements for staff responsibilities and general <br /> information about site safety, environmental practices and regulatory agencies and oversight. <br /> 2.1 DOCUMENT DISTRIBUTION <br /> The O&M manual is a controlled document intended for internal use only. As such, a limited <br /> number of copies are available for use. Future copies may be developed and the distribution <br /> added to the list below. The following entities will maintain a copy of the O&M manual: <br /> • Climax Mill Engineering Office. <br /> • Climax Tailing Operations Office. <br /> • Climax Environmental Department Office. <br /> • URS Office, Denver, Colorado. <br /> • Colorado Division of Reclamation, Mining, and Safety(DRMS). <br /> • Wheeler Office, Englewood, Colorado. <br /> Updates and revisions to the O&M manual will be coordinated through the Climax Mill <br /> Engineering Office so that all copies reflect the changes and they remain identical. <br /> 2.2 ROLES AND RESPONSIBILITIES <br /> It is the responsibility of every Climax employee to perform his or her assignment in a manner <br /> that promotes safety, protects the environment, and is consistent with established responsibilities. <br /> The following personnel are responsible for managing, operating, and maintaining the Climax <br /> tailing dams and appurtenances. <br /> Tailing Operations <br /> Tailing Supervisor—Reed Bennetts(719) 486-7559 <br /> Services Superintendent—Gordon Stinnett(719)486-7616 <br /> Mill Manager—Mark Mansuetti (719) 486-7588 <br /> Tailing&Water(Area 500) Engineering <br /> Tailing Engineer—Tyson Parrott(719)486-7566 <br /> Senior Engineer—Tom Brown (719)486-7635 <br /> Chief Engineer—Ron Valentine (719)486-7565 <br /> Day-to-day operation of 3 Dam is the responsibility of the tailing operators. Currently the <br /> operation of the dam is to be performed by four operators under supervision by the Services <br /> Superintendent.
