Bureau of land Management <br /> Colorado River Valley Field Office <br /> Minerals Inspection Report <br /> During earthwork operations in this area earlier this month,the dozer,with Justin inside,slid down the <br /> embankment and drainage.The dozer is now located at the base of the mill bench in the unnamed drainage <br /> (Photo 4).Justin was able to walk away. He plans to drive the dozer back up to the mill bench via a recently- <br /> constructed trail that climbs up to the southeastern portion of the mill bench. <br /> Deer were observed walking and browsing along the Middle Road during my visit to the operation (Photo 5). <br /> In the equipment yard west of the mill facility, I observed a parked loader,skid steer,excavator,and road <br /> grader, as well as various mine supplies. Duct work within this area is planned to be recycled.A portable diesel <br /> fuel tank was observed sitting on a pallet without secondary containment(Photo 6).The 2015 CRVFO RMP— <br /> Appendix K BMPs state: "MIN-7: Project materials which could be a hazard to public health,safety or resource <br /> value will be stored in appropriate secondary containment. No oil or lubricants will be drained onto the ground <br /> surface."Additionally, per BMP MIN-50: "Standard secondary containment shall hold 110%of the capacity of <br /> the largest single tank it contains and be impervious to any oil,glycol, produced water,or other toxic fluid for <br /> 72 hours. Earthen berms must be compacted and of fine material that will prevent seepage of any spill to <br /> surrounding area." <br /> The mill facility,which was authorized by BLM correspondence dated June 15,2009, contains a small lab on <br /> the western side and crushing,grinding,and bagging equipment on the eastern side within the large main <br /> portion of the building. I observed spilled lubricant with kitty litter on the floor of the main portion of the <br /> building(Photo 7).This area needs general housekeeping. <br /> Topsoil is stored on the far western side of the mill bench (Photo 8).This stockpile appeared to be <br /> approximately at a 2:1 slope and inadequate for future site reclamation.According to the stipulations included <br /> in BLM's approval of Amendment 1,dated July 21, 1989: "The stored topsoil will be graded to a slope of <br /> approximately 4:1 to prevent erosion and to facilitate seeding and establishment of grasses." <br /> A small patch of mullein was observed north of the topsoil storage area. CaIX hires EcoRx to provide weed <br /> management services.A Pesticide Use Plan (PUP) is in place with the BLM. <br /> On the east side of the production bench, I observed two fuel tanks on skids within secondary containment,an <br /> empty and unusable water tank,and an excavator.A portion of highwall on the eastern side was deemed <br /> unsafe by MSHA and CaIX was required to place a berm on the production bench to keep individuals out of the <br /> unsafe area. CaIX intends to expand the quarry towards the northeast in the future and address the safety <br /> hazard by scaling away the unsafe material. If the disturbance is projected to fall outside the permitted <br /> boundary,a Plan of Operations Modification will be required. <br /> On the western side and of the production bench,there is a material stockpile area and portable screening and <br /> sorting equipment. I also observed a poly water container in use near the screening equipment(Photo 9). <br /> After blasting,the rubblized material is pushed down from the mine benches to the production bench and <br /> then transported to the stockpile.The limestone material is transported from the stockpile to the portable <br /> equipment and screened and sorted. Fines are transported to the fines stockpile,which is located on the mill <br /> bench,east of the mill building(Photo 10).Screened and sorted material is transported via conveyer belt <br /> system and ejected onto the mill bench.This material is then transported via another conveyer belt system <br /> into the mill building for further crushing and grinding. <br /> Page 3 of 10 <br />
