Newmont Mining Corporation •■ <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,March 19,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> NewFields personnel observed confirmatory drilling activities and noted any voids that <br /> were encountered. Confirmatory drilling was performed at the following underground <br /> workings: <br /> o UG#6478 (further remediation required) <br /> o UG#6514 (further remediation required) <br /> o UG#6444 (further remediation required) <br /> An excavator worked to remove the shot rock at UG#6617. The working is being <br /> excavated and backfilled in accordance with the project technical specifications. Further <br /> remediation is required. <br /> UG#6520 was backfilled in preparation for the geogrid cap installation. Two layers of <br /> geogrid were placed over the working. Prior to the placement of six inches of select <br /> structural fill over each layer, as built survey was completed. Compaction was achieved <br /> using a smooth drum roller.The working is considered remediated. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint. NewFields <br /> personnel inspected fill material and observed placement and compaction procedures to <br /> ensure that they were in accordance with the project technical specifications. Ambient and fill <br /> material temperatures were monitored and recorded during fill placement to ensure that no <br /> frozen material was placed or compacted in the area. The following structural fill construction <br /> activities occurred during the shift: <br /> y Slope rough grading (cut to fill) in areas bounded by bench stations: <br /> o K6+00 to K8+00 and M2+00 to M4+00 <br /> o N6+00 to N10+00 and M26+00 to M30+00 <br /> o N14+00 to N20+00 and S40+00 to S45+00 <br /> o N14+00 to N22+00 and M34+00 to M42+00 <br /> o 120+00 to 122+00 and K16+00 to K20+00 <br /> Slope rough grading (cut to stockpile) in areas bounded by bench stations: <br /> o K12+00 to K16+00 and M4+00 to M10+00 <br /> o K30+00 to K34+00 and M22+00 to M26+00 <br /> Page 2 <br />
