Newmont Mining Corporation ■ <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQ.A <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,March 26,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> • NewFields personnel observed confirmatory drilling activities and noted any voids that <br /> were encountered. Confirmatory drilling was performed at the following underground <br /> workings: <br /> o UG#6514 (further remediation required) <br /> o UG#6444 (further remediation required) <br /> o Upper Anaconda Mine Complex — UG#6525, UG#6524, UG#6488, UG#6487, <br /> UG#6489 (further remediation required) <br /> • Ames shot the boreholes at UG#6478. The shot rock will be excavated and a <br /> remediation plan will be developed. Further remediation is required. <br /> > Two layers of geogrid were placed on the first "step" of UG#6617. As built survey was <br /> completed prior to the placement of six inches of select structural fill over each layer. <br /> Compaction of the first and second layers was achieved using a smooth drum roller. A <br /> third layer of geogrid will be placed on this "step" and three more geogrid "steps" need <br /> to be placed over the working. Further remediation is required. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint. NewFields <br /> personnel inspected fill material and observed placement and compaction procedures to <br /> ensure that they were in accordance with the project technical specifications. Ambient and fill <br /> material temperatures were monitored and recorded during fill placement to ensure that no <br /> frozen material was placed or compacted in the area. The following structural fill construction <br /> activities occurred during the shift: <br /> • Slope rough grading (cut to fill) in areas bounded by bench stations: <br /> o 120+00 to 122+00 and K18+00 to K22+00 <br /> o K18+00 to K22+00 and M14+00 to M18+00 <br /> o N12+00 to N16+00 and S38+00 to S42+00 <br /> Y Slope rough grading (cut to stockpile) in areas bounded by bench stations: <br /> o M12+00 to M14+00 and S22+00 to S24+00 <br /> o 118+00 to 122+00 and K12+00 to K16+00 <br /> • Fill placement (hauled from stockpile) in areas bounded by bench stations: <br /> Page 2 <br />
