Newmont Mining Corporation <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> Newfields Job No.475.0106.006 <br /> Week Ending Saturday,March 12,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> NewFields personnel observed confirmatory drilling activities and noted any voids that <br /> were encountered. Confirmatory drilling was performed at the following underground <br /> workings: <br /> o UG#6617 (further remediation required) <br /> o UG#6478 (further remediation required) <br /> Confirmatory boreholes at UG#6617 were shot. After the shot, an excavator worked to <br /> remove the shot rock. Further remediation is required. <br /> Shot rock at UG#6520 was excavated. Further remediation is required. <br /> A smooth drum roller compacted the structural fill that was previously placed over the <br /> geogrid at UG#6455 and UG#6456. NewFields personnel ensured that a minimum of <br /> four passes were made to achieve the compaction required by the project technical <br /> specifications. Further remediation is required. <br /> Seven feet of cemented rock fill was placed over the concrete plug at UG#6691. <br /> NewFields personnel observed the cemented rock fill placement. The working was then <br /> backfilled in accordance with the project technical specification. The working is <br /> considered remediated. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint. NewFields <br /> personnel inspected fill material and observed placement and compaction procedures to <br /> ensure that they were in accordance with the project technical specifications. Ambient and fill <br /> material temperatures were monitored and recorded during fill placement to ensure that no <br /> frozen material was placed or compacted in the area. The following structural fill construction <br /> activities occurred during the shift: <br /> Slope rough grading (cut to fill) in areas bounded by bench stations: <br /> o K16+00 to K22+00 and M12+00 to M18+00 <br /> o M44+00 to M46+00 and ADR Haul Road <br /> c 120+00 to 122+00 and K18+00 to K20+00 <br /> o M36+00 to M40+00 and N14+00 to N18+00 <br /> o N14+00 to N22+00 and 538+00 to 548+00 <br /> Page 2 <br />
