Box 875 <br />Westcliffe, Co. 81252 <br />Re: Buffalo Hunter Load, Permit No. M-2016-015 <br />Amendment <br />Application Form <br />1) Confirmed change, "Applicant/Operator's name is "Lucky VI Quarry" <br />Exhibit B- Site Description Rule 6.3.2 <br />2) Confirmed change, Applicant/Operator misused term tailings. There are just stockpiles of waste <br />rock/overburden stockpiles on the site. Confirmed change on Site Description Exhibit B, Mining <br />Plan Map, Mining Plan Exhibit C, Reclamation Plan Map, and Reclamation Plan Exhibit D. <br />Exhibit C -Mining Plan Rule 6.3.3 <br />3) Confirmed that to prevent erosion of topsoil stockpiles, Applicant/Operator will seed permanent <br />stockpiles, in the growing season that they are stockpiled for use of reclamation. Temporary <br />topsoil stockpiles/berms use for safety will be moved outward for mining use as needed and not <br />seeded until they are stored for permanent storage for reclamation, then also seeded( as per <br />NRCS). Confirmed change on Reclamation Plan, Reclamation Plan Map Exhibit D, Mining Plan, <br />Mining Plan Map Exhibit C. <br />4) Confirmed that Applicant/Operator will commit to salvaging at minimum the top 6 inches of soil <br />for reclamation and commit to salvage more growth medium/ topsoil in areas where topsoil is <br />deeper. Confirmed change on Reclamation Plan Exhibit D and Mining Plan Exhibit C. <br />5) Pursuant to Rule 6.3.3(d) Confirmed that the existing north -south access road is proposed to be <br />25' wide X 552' length. Confirmed that the access road north -south is included in the permitted <br />acreage. The necessary improvements for access road will be widened to 25' and ditches cut <br />and graveled. County access off 255 will have 60ft culvert (spec by County) Confirmed change on <br />Reclamation Plan, Reclamation Plan Map Exhibit D, Mining Plan, Mining Plan Map Exhibit C and <br />Site description Exhibit B. <br />
