v. Confirmed the location of man-made structure within 200ft. of the affected area. A <br />brief narrative is included in Exhibit B -Site Description. <br />vi. Confirmed outlined and labeled existing disturbance within or adjacent to the <br />permit boundary. (previously mined area) Re -disturbances of previously disturbed <br />areas, by the proposed miming operation, is included in the permit area and <br />addressed in Exhibit D -Reclamation Plan. <br />b) Reclamation Plan Map: <br />Confirmed shows gradient of all reclaimed slopes (horizontal: vertical) sufficient to <br />describe the post mine topography. <br />ii. Confirmed where vegetation will not be established and the general areas for <br />shrubs and trees planted. <br />iii. Confirmed no ponds in permit area. <br />iv. Confirmed the average thickness of replaced overburden by reclamation area. <br />v. Confirmed the average thickness of replaced topsoil by reclamation. <br />Confirmed the Mine Plan Map and Reclamation Plan Map is signed by qualified person, and dated. <br />11) Confirmed that notice is in local newspaper, notarized statement from newspaper. <br />12) Confirmed proof of mailing notices to all owners of record of the surface and mineral rights of <br />affected land: and owners of record of all land surface owners within 200ft. of the boundary of <br />the affected land. <br />13) Confirmed that Applicant's responses to the issues, have been placed with the application <br />materials previously placed with County Clerk or Recorders Office, and made for public review. <br />14) Confirmed should human remains be discovered during the proposed project activities, the <br />requirements under State law C.R.S. 24-80(part 13) apply and will be followed. <br />
