Newmont Mining Corporation <br />Cripple Creek & Victor Gold Mine <br />Squaw Gulch VLF CQA <br />NewFields Job No. 475.0106.006 <br />Week Ending Saturday, March 05, 2016 <br />NM <br />■ <br />1.0 EARTHWORKS <br />1.1 Underground Working Remediation <br />• NewFields personnel observed confirmatory drilling activities and noted any voids that <br />were encountered. Confirmatory drilling was performed at the following underground <br />workings: <br />o UG#6520 (further remediation required) <br />o UG#6617 (further remediation required) <br />• Confirmatory boreholes at UG#6520 were shot. Further remediation is required. <br />• Underground workings #6692 and #6477 were excavated until competent rock was <br />encountered. An excavator worked to backfill the working in accordance with the <br />project technical specifications. The workings are considered remediated. <br />➢ A concrete plug was placed at UG#6691 (23cy) and UG#6471 (11cy), respectively. Prior <br />to the concrete pour, the rock surface within the shaft was scaled to remove loose <br />material. Seven feet of cemented rock fill was placed over the concrete plug at <br />UG#6471. NewFields personnel observed the concrete pours and cemented rock fill <br />placement. Further remediation is required at both workings. <br />• CAT D6 dozers placed the first layer of select structural fill was over the geogrid at <br />UG#6470 and UG#6531. No compaction activities took place. Further remediation is <br />required. <br />1.2 Structural Fill <br />Structural Fill placement continued within the Squaw Gulch VLF Phase II footprint. NewFields <br />personnel inspected fill material and observed placement and compaction procedures to <br />ensure that they were in accordance with the project technical specifications. Ambient and fill <br />material temperatures were monitored and recorded during fill placement to ensure that no <br />frozen material was placed or compacted in the area. The following structural fill construction <br />activities occurred during the shift: <br />• Slope rough grading (cut to fill) in areas bounded by bench stations: <br />o K8+00 to K14+00 and M2+00 to M8+00 <br />o K26+00 to K32+00 and M20+00 to M24+00 <br />o 122+00 to 124+00 and K18+00 to K22+00 <br />o M10+00 to M16+00 and 518+00 to S26+00 <br />Page 2 <br />
