2015 Annual Reclamation Report Energy Fuels Coal, Inc - Southfield Mine <br />to the Division with materials included in the application for partial bond release No. 3 (SL -03) <br />submitted in October 2015. These 2014 revegetation monitoring data, along with the 2013 <br />revegetation monitoring data, all confirm that the Phase III Revegetation Success Criteria were <br />satisfied on all of the reclaimed areas located at both the Southfield Mine and Southfield <br />Loadout sites. <br />Site Maintenance Activities during 2015 <br />As part of EFCI's regular site maintenance activities and in conformance with any concerns <br />noted by the DRMS in their periodic site inspections, surface water control structures, roads and <br />reclaimed areas were inspected. Based on inspection, the surface water control structures and <br />roads were in very good condition throughout the reporting period, thus requiring only minor <br />maintenance as noted below. In conjunction with final site reclamation and ongoing site <br />maintenance, EFCI completed the following specific activities: <br />• Minor grading of short sections of the mine site entrance road from Pond 5 westerly to <br />the former RDA haul road intersection, and maintained the existing "bar trenches" in this <br />section of road <br />• Minor grading of road system around the RDA <br />• Cleaned/graded road ditch adjacent to intersection of CR92 and the former RDA haul <br />road <br />• During the summer season some of the wattles previously installed on the embankment <br />adjacent to the gravel parking area on the Vento property were cleaned of sediment build- <br />up and straw plugs were compacted in the small holes where water had undercut the <br />wattles. During the summer of 2015, an additional straw blanket was added to the lower <br />portion of the embankment <br />• Wattles and silt fencing previously installed adjacent to the northeast end of the former <br />truck tunnel were cleaned and repaired during the summer season <br />• Repaired subsidence holes on Cribbs -Jackson property during June at the abandoned <br />Mountain View coal mine site. Details of the work performed, including maps, are <br />included in a report submitted by Mergan Ecological Delineations, dated September 14, <br />2015, attached hereto in Appendix B <br />• Removed equipment containing hydraulic oil and drained hydraulic oil from pipes in the <br />former truck tunnel during May. A report dated January 14,2016, was submitted to the <br />Division that provides details of the work and is attached hereto in Appendix C <br />• Demolished the "lab building", at the request of the landowner, that was located near the <br />entrance of the former truck tunnel. All demolition debris were hauled off site for <br />disposal, and the footprint of the building was regraded, seeded, and mulched. <br />• Remove sediment accumulations from both emergency spillway outlets at Pond 4 <br />• Reclaimed small bare area on south side of RDA measuring approx. 25'x 25'. Placed 1 ft. <br />of topsoil on the area, then seed and straw mulch. <br />Page 7 <br />
