Compliance With Mine Plan N - Support Facilities Not On-site <br /> N -Other NA - Special Categories Of Mining <br /> Y -Processing Waste R -Topsoil <br /> COMMENTS <br /> A complete inspection was conducted by Rob Zuber of DRMS on February 24 and 25, 2016. The primary <br /> purpose of the inspection was to perform a bond release inspection(SL-04 Phases I, II, and III),however other <br /> areas were also inspected, and records were checked. The weather was clear and seasonably warm. The ground <br /> was mostly dry. <br /> Other attendees on February 24th(most of day) included: <br /> Al Weaver and Kent Gorham,representing Energy Fuels(EFCI) <br /> Jack Robeda representing the Warnock family and Las Cruces,LTD. <br /> Carin Corley,Daryl Mergen, and W.D. Corley. <br /> At the end of the day on the 24th,Rob Zuber and Al Weaver inspected wells and exploration drill holes. The <br /> Corleys,Daryl Mergen, and Jack Robeda were not interested in these locations. Mr. Gorham left since only one <br /> EFCI representative was deemed necessary for this part of inspection. <br /> On the second day(February 25th),the inspection was conducted by Rob Zuber and Al Weaver only. It entailed a <br /> records check in the Florence office of EFCI,inspection of Pond 5 (unrelated to SL-04), and inspection of some <br /> more locations on Vento property. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage—Acid and Toxic Materials 4.05.8; Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> The inspection included several exploration drill holes(aka bore holes) and monitoring wells. EFCI hired a <br /> drilling company to seal these holes in recent months, subsequent to the inventory of these holes in early 2015. As <br /> part of the SL-04 inspection,they were assessed to confirm that they have been sealed and reclaimed.
