T-2 <br />Example of Notice to Adjacent Landowner/Owner of Man Made Structures <br /> <br />NOTICE OF FILING AMENDMENT APPLICATION <br />FOR COLORADO MINED LAND RECLAMATION PERMIT <br />M-1980-244, CRESSON PROJECT, AMENDED AMENDMENT 11 <br /> <br /><Adjacent Landowner Name> <br /><1234 Main Street> <br /><City, CO 80XXX> <br /> <br />Cripple Creek & Victor Gold Mining Company (“the Operator”) has applied for an <br />amendment to its application for an amendment to an existing 112d Designated Mining <br />Operation reclamation permit from the Colorado Mined Land Reclamation Board (“the <br />Board”). The proposed application amendment is to Amendment 11 to the Cresson Mine <br />and is located in the Cripple Creek Mining District, predominately in Township 15 South, <br />Range 69 West, 6th Prime Meridian. <br /> <br />The proposed application including this amendment will commence as soon as necessary <br />permits are obtained, and the proposed date of completion of associated reclamation is <br />2041. The Proposed future use of the land remains livestock grazing and wildlife habitat. <br />Additional information and tentative decision date may be obtained from the Division of <br />Reclamation, Mining and Safety, 1313 Sherman Street, Room 215, Denver, Colorado <br />80203, (303) 866-3567.
