PERMIT #: M-2004-031 <br />INSPECTOR’S INITIALS: TOD <br />INSPECTION DATE: August 19, 2015 <br />Page 2 of 6 <br />OBSERVATIONS <br />This inspection was conducted as a complete routine monitoring inspection and an inspection to verify compliance <br />with the Temporary Groundwater Mounding Mitigation Plan (temporary plan). This inspection was conducted <br />by Tony Waldron, Wally Erickson, and Tyler O’Donnell of the Division of Reclamation, Mining and Safety <br />(Division). Aggregate Industries - WCR, Inc., the Operator, was represented by Connie Davis and Mike Refer <br />during the inspection. Steve O’Brian with Environment, Inc., the consultant representing Fred Orr, the adjacent <br />landowner, was present during the inspection. <br />The Hazeltine Mine is located in Adams County approximately 3 miles south of Henderson Colorado, just north <br />of 104th Ave. and east of the South Platte River. The Hazeltine Mine is a 203.90-acre 112 Construction Materials <br />Reclamation Permit. The approved post-mining land use is developed water resource. <br />At the time of the inspection it was sunny. The majority of the mine was dry, the northeastern corner of the <br />Hazeltine Mine had a small pond in the bottom of the pit. The site was active during the inspection. The mining <br />was progressing into the southeastern corner of the pit, phase 2. The material being mined during the inspection <br />was being transported off-site, via a conveyor to the adjoining permit for processing. All processing was taking <br />place within the permit boundary of Cooley Reservoir & Fulton Wildlife Area (DRMS Permit No. M-1999-034). <br />It appears that the majority of the site had been mined. The Operator’s representative stated that the site would <br />be mined out within the next couple of months and final reclamation would begin shortly after the completion of <br />mining. <br />Backfilling and Grading: <br />The majority of the site appears to have been graded to 3H:1V (Photo 1). The Operator was in the process of <br />adding additional backfill material to portions of the pit slopes to bring them to 3H:1V (photo 2). The Operator’s <br />representative stated that the 2013 flood damage to the pit slopes and slurry wall would be repaired by the end of <br />2015. There were sections where the slurry wall did not completely seal during the initial construction of the <br />slurry wall. The Operator’s representative stated that the damaged or improperly sealed portions of the slurry <br />wall would be repaired shortly. The Operator had equipment and materials mobilized at the site to make the <br />appropriate repairs to the slurry wall. <br />Financial Warranty: <br />The current amount of financial warranty the Operator has on deposit with the State is $670,602.00. <br />The reclamation cost estimate for this pit was updated with this inspection. It was determined that the current <br />financial warranty amount is adequate to conduct the current approved reclamation plan. <br />Hydrologic Balance: <br />During the inspection the Division, Operator, and Landowner’s representative went to the Orr property to observe <br />the current conditions. The Division wanted to observe and evaluate the effectiveness of the Temporary <br />Groundwater Mounding Mitigation Plan (temporary plan). The temporary plan was required by an April 20, <br />2015, Mined Land Reclamation Board (Board) Order. The temporary plan was submitted on May 21, 2015, as <br />technical Revision No. 4 (TR-04). TR-04 was approved on June 15, 2015. TR-04 proposed the installation of <br />two 18 inch CMP pipes, going through the slurry wall, thus draining into the Hazeltine Mine Pit (photo 3). The <br />two 18 inch pipes were set at an approximate elevation of 5,040.78 feet, draining the Bull Seep and groundwater <br />that had mounded against the slurry wall, thus expressing at the land surface and collecting into the Bull Seep. <br />During the inspection the Division walked through the majority of the Orr Property. Prior to the commencement <br />of the temporary plan there was a small pond in the western portion of the Orr property.
