My review of the permit with respect to these rules follows. Adequacy review questions are
bulleted (♦).
General Document Issues:
A. DRMS Jan 2016 Where the abridged permit references Maps and Appendices, these referenced
documents should be included in the Abridged Permit. This would be best accomplished with a
M R.
♦ Please submit a MR to incorporate referenced appendices, maps, and other information into the
abridged permit so that it functions as a stand alone document.
B. DRMS Jan 2016: Revised Pages for PR1 issued September of 2015 carry the revision date of
September 2014.
♦ Please advise if this is the most recent revision date for pages:
1. page 1-1; pages 3-1 thru 3-6; pages 3-9 thru 3-12;
2. pages 4-1 thru 4-22;

Rule: 2.03.4 (1-4) Identification of Interests
Rule 2.03.4(1-4) has been adequately provided in the abridged permit and includes entity type
ownership and contact information.
Rule: 2.03.5 (1-3) Compliance Information
This is a review of the compliance information required by Rule 2.03.5. This information has been
adequately provided in section 2.03.5 pg. 3-7 of the abridged permit.
Rule: 2.03.6 (1-2) Riaht of Entry
This is a review of the requirements for right of entry, and includes a permit boundary legal
description, and ownership information.
Rule 2.03.6 (1-2) has been adequately addressed in Appendix 3-2 of the abridged permit and
includes information on new surface ownership as of June 2015 as well as an executed right of entry.
