The <br />Project Manager, Ms. Andrea Meduna, PE, or Senior Geotechnical Engineer Brett Byler, <br />PE, made periodic site visits to resolve technical issues and to review Amec Foster <br />Wheeler activities. Appendix C presents a summary of the Amec Foster Wheeler staff <br />on-site during the construction period. <br />Representatives of CC&V, Ames, ECA, and Amec Foster Wheeler conducted pre- <br />construction, daily, and weekly coordination meetings throughout the construction <br />period. <br /> Daily Construction Summary Reports <br />Amec Foster Wheeler prepared daily construction summary reports as well as <br />photographic documentation (Appendix B) throughout the project to document the key <br />elements of construction and the CQA activities. On-site CQA personnel and the Project <br />Manager discussed construction activities on a regular basis; in addition, field personnel <br />prepared daily observation reports. Amec Foster Wheeler submitted these daily <br />summary reports to CC&V. Daily reports are not included in this report, but were <br />available for the Colorado Division of Reclamation, Mining and Safety (DRMS) review <br />during the course of the project. <br /> Weekly Construction Summary Reports <br />Amec Foster Wheeler prepared weekly construction summary reports which were <br />submitted to CC&V and to the DRMS. Weekly reports from the end of September <br />through mid-December 2015 are presented in Appendix D. Weekly construction <br />summary reports are continued from the Squaw Gulch VLF Phase 1 (9,450’ to 9,550’ <br />Bench) report by Amec Foster Wheeler.
