Cripple Creek & Victor Gold Mining Company <br />Squaw Gulch VLF Phase 1 <br />(9,550-foot Elevation Bench to Geomembrane Panel P1635) <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 2016 <br />Project No.: 74201125N0 Page 3 <br />America manufactured the geomembrane used to construct Phase 1. ECApplications, <br />Inc. (ECA) installed geomembrane liner from June 2015 through November 2015 for <br />installation of geomembrane for this report. Messrs. Ron Roberts, Ron DiDonato, and <br />Jeff Gaul represented CC&V as the Construction Manager and Construction <br />Superintendents, respectively. All quantities and testing frequencies presented in this <br />report are current as of December 2015, which represents completion of the Project, as <br />detailed in this report. <br />1.2 Construction Quality Assurance/Construction Quality Control <br />Amec Foster Wheeler provided CQA and CQC monitoring and testing services, and <br />prepared this report describing the results for the Project. Record of construction <br />drawings and supporting CQA monitoring and testing documentation are presented in <br />appendices attached to this report. <br /> CQA Monitoring of Earthworks <br />Amec Foster Wheeler provided CQC and CQA testing and monitoring of earthworks for <br />the Project. CQA testing and monitoring of earthworks was provided for in-place <br />materials in accordance with the AMEC Earthworks CQA Plan (Section 1400.1 of the <br />project technical specifications; see Appendix E). The earthworks material specifications <br />for this project are summarized in Table 3. CQC earthworks test results were provided <br />to support the contractor during the manufacturing of materials and prior to material <br />placement. The CQC test results are not included in this report as these tests were <br />intended to assist the contractor in materials preparation rather than record testing. All <br />CQA testing results are included. <br />Amec Foster Wheeler provided CQA laboratory and field testing, and monitored <br />earthworks construction activities as follows: <br />• Amec Foster Wheeler monitored topsoil stripping, site grading, slope re-contouring, <br />and structural fill (SF) placement, to include lift thickness, moisture conditioning, <br />temperatures, compactive effort, in-place nuclear density testing, or monitored <br />placement according to the method specification, and use of suitable SF materials. <br />The materials were monitored during placement to confirm that the subgrade surface <br />was unyielding, free from unsuitable and organic materials and was acceptable for <br />soil liner fill (SLF) placement. Amec Foster Wheeler performed CQA laboratory <br />testing of grain size and Atterberg limits. <br />• Amec Foster Wheeler monitored construction of the Leak Detection System (LDS), <br />including verification that Ames excavated the trench to the minimum required <br />dimensions and placed materials as specified. Laboratory testing performed by <br />Amec Foster Wheeler consisted of grain size and Atterberg limits of the Leak <br />Detection Fill (LDF) placed in the LDS.
