ATTACHMENT A <br /> October 10,2008 <br /> 0847: WWL and GSD arrive at Whirlwind Mine,monitoring well site MW-W1.Weather is broken,periodic <br /> light rain,cool and breezy. <br /> 0850: DTW in well is 73.46 BTOC. <br /> 0905: GSD begins tripping drill rod into borehole. <br /> 0915: GSD begins to inject and flush out borehole. Weather is cold with sporadic rain and snow. With <br /> injection,borehole yields 5 to 10 gpm;water is turbid,tan/brown color;mostly fine-grained sediment <br /> and some chips. <br /> 1009: Stop injecting,continue to flush hole with ambient groundwater;no change in water quality. <br /> 1100: Begin well construction.Hand-feed approximately 4 gallons of washed 3/4-inch gravel to bottom of <br /> borehole to stabilize and raise bottom to preferred screen interval.Tagged bottom with 120 ft of 2-inch <br /> Johnson,Schedule 40,PVC well casing minus stick-up of 52-inches for hole depth of 115.7 ft. WWL <br /> instructs GSD to install one 5-ft long sump of Johnson 2-inch PVC non-slotted casing with an end cap; <br /> 50 ft of Johnson 2-inch PVC 20 slot(0.020—inch)well screen;and 60 ft of Johnson 2-inch PVC non- <br /> slotted well casing. Final well construction specifications are shown in Figure 1. Five centralizers <br /> were used and spaced at 10 ft,30 ft,50 ft,70 ft,and 90 ft from the bottom of the well casing. Total <br /> well casing used was 118.2 ft including stick-up of 2.7 ft. <br /> 1255: All well casing in borehole;DTW is 74.31 ft BTOC(steel). <br /> 1300: Trip down with 84 ft of 1 '/4-inch steel tremie pipe(pre-flushed with fresh water on surface). <br /> 1318: Begin adding No.8-12 Colorado Silica Sand®to tremie with fresh water. <br /> 1439: All 16 bags of 8-12 sand on site were tremied into the borehole annulus. Tag sand at 69.5 ft BTOC. <br /> Top of well screen is calculated at 63 ft BTOC.More sand is required as washout in borehole must <br /> have consumed more sand than anticipated. GSD arranged for WWL to bring additional No.8-12 sand <br /> from Grand Junction tomorrow. <br /> 1540: GSD off site for the day. WWL measures DTW at 37.4 ft and is falling at a rate of approximately 0.1 <br /> ft/40 seconds.WWL stores well cuttings at main dry house at Whirlwind Mine with approval of Bruce <br /> Norquist,Engineer. Surveying of the well was discussed also. <br /> 1550: WWL off site for the day. <br /> 1730: WWL picks up No. 8-12 sand in Grand Junction. <br /> October 11,2008 <br /> 1215: GSD arrive at Whirlwind Mine,monitoring well site MW-W 1. <br /> 1220: WWL arrives on site.Weather is broken,cool,and windy. <br /> 1222: DTW is 73.55 ft BTOC(PVC).
