ATTACHMENT A <br /> Energy Fuels Resources Corporation <br /> Whirlwind Mine <br /> Monitoring Well MW-W1 Construction <br /> Field Log Summary <br /> Gordon Smith mobilized his rotary Speed Star drilling rig on October 7 h,2008. Well construction occurred <br /> from October 9—12,2008. The following information summarizes daily well construction activities. <br /> October 9: <br /> 830: WWL(Bruce Smith)arrives at Whirlwind Mine,monitoring well MW-W 1 site. Weather is clear,cool <br /> to mild. <br /> 0844: Gordon Smith Drilling(GSD)arrived on site. GSD crew consists of Gordon Smith and Nevada Smith. <br /> WWL confirmed with Dick White(EFR)by way of phone,the monitoring well site location, <br /> specifically the distance from County Road 5110. Dick also had provided a map and hydrogeologic <br /> information from nearby exploration holes that had been drilled recently. <br /> 0950: GSD commences drilling operations using air;no water was injected at this time. The well was <br /> collared in the Brushy Basin Member of the Morrison Formation.The geologic log(Attachment B) <br /> shows that rock types encountered were primarily variegated mudstones,claystone,siltstone,and <br /> sandstone. GSD first reported damp cuttings at a depth of approximately 30 feet(ft)and again at <br /> approximately 78 ft. At a depth of 80 ft,WWL instructed GSD to let the hole set a few minutes and <br /> follow with air to assess if water is accumulating in the borehole. This resulted in 10 to 50 gallons of <br /> water being blown out of the hole. A repeat of this had the same result. WWL instructed GSD to drill <br /> to 100 ft and then water yield would be re-assessed. <br /> 1200: Depth was 102.5 ft.The hole was allowed to set approximately 10 minutes and then blown out.Again, <br /> approximately 10 to 50 gallons was produced on the surface. The boring consistently produced <br /> approximately 1 gpm for 10 minutes after the well storage was blown from the hole.WWL concluded <br /> that the boring was producing sufficient water in the open interval depth to potentially serve as a <br /> downgradient monitoring well. Mr.Frank Filas of EFR was contacted and agreed with the decision to <br /> screen the well at the first significant groundwater encountered.WWL instructed GSD to drill to a total <br /> depth of 120 ft. This depth was selected to potentially intercept additional groundwater and to add <br /> sufficient screen length and well storage to the constructed well.GSD set up to inject water with air. <br /> 1305: Total depth of the boring is 120 ft.The hole was flushed for 15 minutes and GSD began to set up to <br /> drillout surface casing. <br /> 1405: Finished setting and cementing 6-inch steel surface casing to depth of 3 ft below ground surface. GSD <br /> will let cement cure and will set well casing tomorrow. <br /> 1430: GSD off site for day. <br /> 1510: WWL measured depth to water(DTW)in well at 73.84 ft below top of steel casing(BTOC).
