OPERATORS <br /> SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br /> WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br /> Possible Violation is indicated,you will be notified under separate cover as to when the Mined Land <br /> Reclamation Board will consider possible enforcement action. <br /> INSPECTION TOPIC:Availability Of Records <br /> PROBLEM/POSSIBLE VIOLATION: Problem: The Operator's name has changed from Carma Bayshore, LLC to <br /> Barefoot, LLC according to the Colorado Secretary of State. <br /> CORRECTIVE ACTIONS:The Operator must submit a Succession of Operator form, as required by Rule 1.12, to <br /> update the Permittee for the site by the corrective action date. <br /> CORRECTIVE ACTION DUE DATE: 2/15/16 <br /> OBSERVATIONS <br /> The St. Vrain Lakes site was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br /> (Division/DRMS) as part of the Division's monitoring inspection program. Mr. Marc Savela with Bayshore, LLC <br /> (Bayshore) was notified of the inspection, but was not present during the inspection. The site was previously <br /> inspected by the Division on August 30, 2012. <br /> The Operator's name has changed from Carma Bayshore, LLC to Barefoot, LLC since the last inspection. As <br /> cited as a problem, the Operator must submit a Succession of Operator form, as required by Rule 1.12, to <br /> update the Permittee for the site. The Succession of Operator form is available on the Division website at <br /> http://mining.state.co.us/SiteCollectionDocuments/TransferMineralPermitpackage.pdf <br /> The Division received a request for a technical revision to update the seed mix for the site from LT <br /> Environment, Inc.
