SWMP Records: <br />The permittee shall retain copies of the facility SWMP, including any modifications made during the term of this <br />permit, documentation related to corrective actions taken, all reports and certifications required by this permit, <br />monitoring data, and records of all data used to complete the application to be covered by this permit, for a <br />period of at least 3 years from the date that coverage under this permit expires or is terminated. <br />3. Additional ReDortine <br />In addition to the reporting requirements stipulated in this Part, the permittee is also subject to the standard <br />permit reporting provisions of Part II of this permit. <br />4. Signatory and Certification Requirements <br />a. All reports and other information required by the Division, shall be signed and certified for accuracy by the <br />permittee in accord with the following criteria: <br />i. In the case of corporations, by a responsible corporate officer. For purposes of this section, the <br />responsible corporate officer is responsible for the overall operation of the facility from which the <br />discharge described in the form originates; <br />ii. In the case of a partnership, by a general partner; <br />iii. In the case of a sole proprietorship, by the proprietor; <br />iv. In the case of a municipal, state, or other public facility, by either a principal executive officer, or <br />ranking elected official. For purposes of this section, a principal executive officer has responsibility for <br />the overall operation of the facility from which the discharge originates; <br />v. By a duly authorized representative of a person described above, only if: <br />1) The authorization is made in writing by a person described in i, ii, iii, or iv above; <br />2) The authorization specifies either an individual or a position having responsibility for the overall <br />operation of the regulated facility or activity such as the position of plant manager, operator of a <br />well or a well field, superintendent, position of equivalent responsibility, or an individual or <br />position having overall responsibility for environmental matters for the company. (A duly <br />authorized representative may thus be either a named individual or any individual occupying a <br />named position); and, <br />3) The written authorization is submitted to the Division. <br />b. If an authorization as described in this section is no longer accurate because a different individual or <br />position has responsibility for the overall operation of the facility, a new authorization satisfying the <br />requirements of this section must be submitted to the Division prior to or together with any reports, <br />information, or applications to be signed by an authorized representative. <br />c. The permittee, or the duly authorized representative shall make and sign the following certification on all <br />such documents: <br />
