PERMIT #: M-1980-244 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: November 10, 2015 <br /> <br /> <br />Page 3 of 8 <br />SGVLF Inspection: <br />Photo 7 shows an overview of the SGVLF liner construction status on November 10, 2015. Ponded water was <br />still observable in the PSSA and an extensive area of placed geomembrane liner and drain cover fill (DCF) was <br />observed. Mr. Hanks expected the ponded water t o be backfilled/covered soon and noted that if any DCF <br />sloughing on the liner occurs, CC&V will follow approved methods ensure any compromised liner is repaired <br />and certified in accordance with project specifications. <br /> <br />Water levels: The inspection continued as the Division visited each of the high and low solution collection <br />system transducers and recorded water level values. The recording sheet is included as Attachment A, and the <br />values are summarized below in the Transducer Readings. Mine Low Volume Solution Collection System (LVCS) <br />inspection log books were observed in the Phase IV and V pump shelter buildings. The Phase IV Low Volume <br />Collection System transducer # 1 displayed an erratic water level. Mr. Hanks stated that typically indicates the <br />pump is cavitating. <br /> <br />The North and South Arequa Gulch underdrains were inspected. The South Underdrain discharge was <br />determined to be 30.0 gpm. The North Unde rdrain was dry. The A35 pumpback line was dry. The B63 <br />pumpback was flowing at 0.2 gpm.
